Fees
My fee is $150 per hour for all therapy services. I am a preferred provider in the following insurance plans:

Uniform Medical Plan
First Choice Health
If you have other insurance coverage, I can provide a
receipt which you can use to submit for reimbursement
as your plan permits.

Forms
Once you have made an appointment with me, I will ask you to complete a couple of forms before our first meeting. If
you would like to fill them out before your appointment, please print out each one, complete them, and bring them with
you. To print them you will need to be able to read the files with Adobe Reader. Most computers have the necessary
program, but if you need to, you can download a copy from the link at the bottom of the page.
